PLEASE NOTE: As of 4/4/2023, the US Department of State will NOT allow our office to accept passport applications if the applicant's travel is within 9 weeks of the application date. Customers will need to call the National Passport Information Center (1-877-487-2778) within 14 calendar days of their scheduled travel (or within 28 calendar days if they need a foreign visa) in order to make an appointment for passport  services at a U.S. Department of State Passport Agency.  

Appointments Recommended for Passport Services

Save time and schedule an appointment to apply for a passport at our downtown Fort Myers office. Anyone applying for a passport must appear in person with all of the requirements listed in the menus below.   

Our Cape Coral branch office does not provide passport services as the U.S. Department of State only allows a certain amount of acceptance facilities within each jurisdiction. For the closest facility to you, please visit Where to Apply for a Passport Nationwide (state.gov).

Appointment Requirements

1. ALL applicants, including children, MUST be present in our office when applying for a US Passport.

2. Passport application must be printed on single pages (not two sided) and completed (but NOT signed) legibly with black ink.

  • DO NOT USE white out or cross out errors on the application. Corrections made with white out, crossing out the incorrect information or writing the correction on top of an error are not acceptable.

3. Must have Proof of Citizenship

  • US Citizens- original or certified copy of birth certificate (long form with both parents’ names listed)
  • Naturalized US Citizens- original naturalization certificate or certificate of citizenship
  • Applicants under age 18 naturalizing through a parent- original or certified copy of foreign birth certificates with a certified English translation of the foreign birth certificate, a permanent resident card and one parent’s original naturalization certificate

4. Must have Identification

  • Applicants 18 years or older- Valid government issued photo identification
  • Applicants 16 to 17 years old- Valid government issued photo identification of one parent and of applicant (MUST show at least one legal parent or legal guardian is aware of passport application)
  • Applicants under 16 years old- Valid government issued photo identification of each parent listed on child’s birth certificate (Both parents/guardians must authorize the issuance of the child's passport. The best way to do this is for both parents/guardians to go with the child in person to apply for the passport. If both parents cannot appear in person, please visit travel.state.gov "Show Parental Consent" section for options)

5. Must bring copies of front AND back of Photo Identification and Citizenship documentation. (Clerk's Office offers copy services for a fee)

  • Copy of front and back of photo identification on 8 ½ x 11 sheet of paper
  • Copy of front and back of citizenship document on 8 ½ x 11 sheet of paper

6. Must have 2x2 Passport Photo that meets US Department of State requirements. (Clerk's Office offers passport photo services for a fee)

Acceptable Forms of Payment

Make checks payable to:

  • U.S. Department of State (check or money order ONLY)
  • Clerk of Courts (check, cash or money order) - Photocopy fee may apply

Once the application is completed and mailed, we no longer have access to your information. Any questions after the application is submitted, must be directed to the US Department of State. by calling 1-877-487-2778 from 6 a.m. - midnight, Monday through Friday (excluding federal holidays) or visit their Application Status page.

Application Requirements Under the Age of 16

1. Evidence of U.S. Citizenship - certified U.S. birth certificate or previously issued passport (passport cannot be damaged)

2. Evidence of Parental Relationship - certified U.S. birth certificate of minor (must list parent’s names)

3. Photo Identification - Valid Driver’s License or current undamaged passport (if issued less than 15 years ago) copies of both the front and back of the parents ID must be submitted with the application. (copy fees apply)

4. Parental Consent - Both parents/guardians must appear with the minor child or submit the Form DS-3053 Statement of Consent from the non-applying parent.

5. Passport Photo - Must be color 2-inch x 2-inch passport photo (Clerk's Office offers passport photo services for a fee)

6. Application Forms - DS-11 application form must be completed

Passport Fees - Separate checks or money orders are required for each applicant. See Fees Schedule for current fees and accepted payments. There is a .15-cent photocopy fee per sheet for driver license and evidence of citizenship.

Application Requirements Ages 16 and 17

Applicants ages 16-17 with their own identification can apply for a passport by themselves. However, it is recommended that at least one parent appear in person with the child to identify him/her and to show parental awareness.

Although two-parent consent does not apply to children aged 16 or 17, per the U.S. State Department regulations, a passport will generally not be issued to a child if the parent or legal guardian has notified the department in writing that he/she objects to issuance of the passport.

Examples of Parental Awareness

  • Having a parent accompany the child when applying, or
  • A signed statement consenting to issuance of a passport from at least one parent (should be accompanied by a photocopy of that parent’s ID).

Photocopies and notarized copies are not acceptable:

Application Requirements Ages 18 and Older

You must apply and complete the Passport Application Form DS-11 12-2010 in person if:

  • You are applying for your first U.S. passport; or
  • Your previous U.S. passport was issued when you were under age 16; or
  • Your previous U.S. passport was lost, stolen, or damaged; or
  • Your previous U.S. passport was issued more than 15 years ago; or
  • Your name has changed since your U.S. passport was issued and you are unable to legally document your name change.

You must bring the following when applying:

    1. Evidence of U.S. Citizenship- Certified U.S. birth certificate or previously issued passport (passport may be expired, but cannot be damaged)

    2. Photo Identification - Valid Driver’s License or current undamaged passport (if issued less than 15 years ago) copies of both the front and back of the parents ID must be submitted with the application. (copy fees apply)

    3. Passport Photo - Color 2-inch x 2-inch passport photo (Clerk's Office offers passport photo services for a fee)

Renewals

You can renew by mail using Form DS-82 if your most recent passport:

  • Is submitted with your application
  • Is undamaged (other than normal "wear and tear")
  • Was issued when your were age 16 or older
  • Was issued within the last 15 years
  • Was issued in your current name (or you can document your name change with an original or certified copy of your marriage certificate, divorce decree, or court order)

If any of the above statements do not apply to you, you must Apply in Person using Form DS-11.

To Renew by Mail:

    1. Complete Form DS-82

    2. Submit your most recent US Passport

    3. Include Name Change Documents if applicable

    4. Provide a 2-inch x 2-inch passport photo (Clerk's Office offers passport photo services for a fee)

    5. Check or money order payable to U.S. Department of State for $130

    6. Mail your completed application and payment with tracking via United States Postal Service (Address for mailing varies as to where you are located, see Department of State renewal page)
