All customers are required to print legibly in black ink and complete all of the information requested on the passport application. Internet downloaded DS11 application forms should be printed in black ink and printed as single pages.
If you are traveling or need your passport in order to get a visa within the next 2 weeks, please call the National Passport Information Center at 1-877-487-2778.
Expedite Service is offered if you need your passport quicker than the routine service can provide. Additional fees apply: a $60.00 expedite fee and a $14.85 postage fee to be added to the State Department passport fees per applicant. In addition, one (1) $17.50 overnight stamp will need to be purchased at the Post Office.
If you expedite a passport application, we recommend you apply in the office before 4:30 p.m. Monday through Friday.

First-time passport book or card applicants may apply for a passport book or card at the Clerk of Courts' Recording Office which is located at 2115 Second Street in the County Administration Building in downtown Fort Myers, between the hours of 8:00am and 5:00pm, Monday through Friday. The application process on average will take each applicant approximately 30 minutes to complete.

If you expedite a passport application, we recommend you apply in the office before 4:30 p.m. Monday through Friday. Prior to applying for a passport, you must obtain an overnight express mail stamp for $17.50 from the Post Office (up to 7 applications).
Proof of Citizenship: A previous U.S. Passport (mutilated, altered, or damaged passports are not acceptable as evidence of U.S. citizenship). Or a certified birth certificate issued by city, county, or state. A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office, which must be within 1 year of your birth. Please note that some short (abstract) versions of birth certificates may not be acceptable for passport purposes.
Proof of Identity: You may prove your identity with one of the following, if you are recognizable: previous U.S. Passport (mutilated, altered, or damaged passports are not acceptable as evidence of U.S. citizenship); Naturalization Certificate; Valid Driver License, Valid Government ID; Valid Military ID (military and dependents)
Photographs: Both photographs must be 2X2 inches in size; identical; taken within the last 6 months showing current appearance; color; full face front view with a plain white or off white background; between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head; taken in normal street attire; no hats or headgear that obscures the hair or hairline.
Passport Card: Adult (age 16 and older) $20.00; Child (age 15 and under) $10.00. The new wallet-size U.S. Passport Card is a travel document that can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry. Note that it cannot be used for international air travel.

You may file for a renewal if you already have a passport that is not damaged; and you received it within the last 15 years; and you were at least age 16 when it was issued; and you still have the same name as in the passport (or you can legally document your name change). Renewals are handled through the mail, by the applicant. You must complete the DS-82 renewal application by mail, which is available in our offices, online at http://travel.state.gov. or can be received by sending a self-addressed, stamped envelope. You will need your most recent passport, two identical passport photos, and a $75.00 fee payable to the U.S. Department of State.
To obtain a renewal form, send a stamped, self-addressed envelope with your request for a renewal form to the Clerk of Courts, Recording Office, Post Office Box 2278, Fort Myers, Florida 33902. You can also download the DS-82 passport renewal form.

To change the name on your passport within 1 year of issuance, there is no fee and handled through the mail. You must complete the DS-5504 form, which is available in our offices, online at http://travel.state.gov, download the DS-5504 form, or can be received by sending a self-addressed, stamped envelope to the Clerk of Courts, Recording Office, Post Office Box 2278, Fort Myers, Florida 33902.
You must provide proof of the name change, i.e., certified marriage certificate, certified copy of the court order as well as two new color passport photos and your old passport. If your name has changed by other means and/or it has been more than one year from the date of issuance of the passport, you must complete the DS-82 renewal application and pay all applicable fees, click here to download the DS-82 form.
